Overview:Presbyterian Villages of Michigan, a faith-based,
premier nonprofit organization providing senior housing and
services in Michigan, has a great opportunity to join our senior
affordable housing operations team as the Lead Service Coordinator,
QA, and Wellness. We invite you to help us create new possibilities

outstanding and innovative colleague will have six (6) or more
years of successful experience in service coordination, quality
assurance, with seniors. Bachelors degree in Social Work,
Gerontology, Psychology, Counseling, or a related health care
field; masters degree is preferred but not required. Two to four
years related experience in social service delivery, 6 years as
Service Coordinator, or Quality Assurance. Understanding of Fair
Housing, HUD, QA, Enhanced Service Coordination, Wellness

Service Coordination, QA, & Wellness is responsible for provide
oversight, leadership, and quality assurance to PVM Service
Coordinators. Provide technical assistance and guidance on the role
of the service coordinator. Assist service coordinators in
developing supportive services plans and property-wide profiles.
Work with PVM leadership to develop outcomes and appropriate
outcome measurements in alignment with PVM strategic plan. Assures
regulatory compliance and conducts quality assurance surveys,
results, and recommendations. Organize and facilitate meetings with
system-wide wellness staff, and develop wellness plan. Provide
oversight of the Wellness Partnership Coordinator position and
grant. Responsibilities:Lead Service CoordinationProvide leadership
and supervision of clinical practice to PVM Service Coordinators in
the daily operations of their job responsibilities.Lead and manage
the Grant Solution reporting requirementsLead and coordinate the
annual service coordinator budget processResponsible for onboarding
and training of new Service Coordinator employees Reviewing service
coordinators files for compliance, including onsite and
virtualProviding ongoing technical assistance and guidanceReviewing
the supportive services plan at each Village annuallyHelp service
coordinators establish and sustain partnerships, including local
partnerships and PVM strategic partnershipsRead current literature,
talk with colleagues, continue education, or participate in
professional organizations or conferences to keep abreast of
developments in the field. Provide/coordinate continuing education
on topics such service availability, application procedures,
resident rights, health/safety/fitness, issues relating to aging
etc. Provide Administration with regular status reports of PVM
communities overallConduct annual Resident and Administrator
Surveys as part of the Quality Assurance process Work with
administrators on Service Coordinator plans for improvement and
monitor.Quality AssuranceAccountable for regulatory compliance
monitoring, including overseeing and coordinating the reporting of
all PVM activities and the results of their QA reviews to Housing
Operations leadership and HUD on an annual basis, including the
submission of the Standards for Success reportsSystem-wide Resident
Wellness Convene and facilitate quarterly meetings of system-wide
Villages wellness, enrichment and activities staff for educational
purposes, planning, sharing best practices and general
networking.Serve as a resource and advisor to system-wide Villages
wellness, enrichment and activities staff.Monitor and maintain an
inventory of wellness, enrichment and activities programming at
each Village system-wide.Oversee Wellness Partnership Coordinator
position and grant. Ensure that metrics and goals are met and
provide summary reports to the Foundation, Housing Operations. Work
collaboratively with the PVM Technology Department on the
Telehealth Suites implementation and reporting.In-person visit to
each Village at least once a year to assess overall wellness,
enrichment and activities programming. During the visit meet
together with the administrator, service coordinator, wellness
staff (if any) and a representative from the Village
Board.Coordinate the development of system-wide policies,
procedures, etc. related to wellness, enrichment and activities
programming.In-person meeting quarterly with Housing Directors and
Executive Directors to review status of system-wide wellness,
enrichment and activities programming.Prepare a system-wide annual
report that summarizes the status of wellness, enrichment and
activities programming. OtherAttend and participate in all
appropriate meetings and training opportunities in order to keep
informed on current information and skill development. Follow
established policies and procedures including but not limited to:
Presbyterian Villages policies and procedures. Safety policies and
procedures. Federal, state and local regulations. All other duties
as assigned Qualifications:Education : Bachelors degree in Social
Work, Gerontology, Psychology, Counseling, or a related health care
field; masters degree is preferred but not required. Experience :
Six or more years related experience in social service delivery, 6
years as Service Coordinator, or Quality Assurance. Understanding
of Fair Housing, HUD, QA, Enhanced Service Coordination, Wellness
Initiatives. Computer Skills Must have high level of PC literacy
particularly Proficient with MS Outlook, MS Word, and MS
PowerPoint.Previous experience in CareGuide, AASC online, RealPage
or other similar software required
