GENERAL SUMMARY: + Responsible for the provision of social work services to patients and their families. PRINCIPLE DUTIES AND RESPONSIBILITIES: + Conducts initial psychosocial interviews with patients and families to assess baseline level of functioning, comprehension of illness and its ramifications, and impact of individual/family history upon current and future psychosocial functioning. + Engages patients and their families as part of the care team through advocacy, ongoing communication, health education, identification of resources and service facilitation. + Provides ego-supportive counseling and interventions to patients and families to maximize ongoing psychosocial functioning. + Provides crisis intervention to patients, families and medical staff as needed. + Functions as a patient advocate and as a liaison between the patient and multiple care delivery systems. Assists patients with linkage to hospital and community resources based on needs assessment. Provides information and referral services as needed. + Participates in quality improvement meetings and activities. + Participates in comprehensive inter-disciplinary team (IDT) meetings with appropriate documentation on IDT template of psychosocial assessment and by creating personalized, patient -centered plan of care. + Demonstrates professional judgment and critical thinking and in assessing department dynamics and responds in a manner that will contribute to smooth functioning of the department. + Displays the ability to perceive problems and assist in developing a plan to cope or resolve the problem. + Evaluates the patient and family needs for long term care when appropriate, and level of care changes. + Explores and evaluates the financial circumstances of the patient and family providing appropriate education and referrals as indicated. + Provide patient education about the benefits of Advance Care Planning and help facilitate patient completion of an Advance Directive so patient's wishes and decisions are understood and considered by the care team. + May organize and facilitate group support meetings for patients, families, or care givers. + Assesses environment resources and obstacles to maintaining safety. + Records psychosocial assessments, update assessments, and progress notes in accordance with established recording guidelines. + Participates in process improvement activities designed to reduce risk, inclusive of data collection, analysis and follow-up intervention activities. + Conduct PHQ2/9 Depression Screening with patients to assess for the presence and severity of depression where applicable. Provide appropriate resources and interventions to ensure patient safety and follow-up where applicable. + Demonstrates the ability to communicate effectively to staff, management and/or medical employees. + Participates in professional development activities, such as attending social work meetings, relevant conferences, professional memberships, and professional reading. + Works closely with other unit social workers to facilitate patient transfer and information flow between units to develop appropriate patient care plans where applicable. + Performs other duties as assigned EDUCATION/EXPERIENCE REQUIRED: - Masters of Social Work degree required. - Must be a Licensed Master's Social Worker (LMSW). - ACSW and previous experience in a healthcare setting preferred. - Demonstrates understanding of the impact of involvement, personal attitude toward illness, handicaps, death and loss, on the quality of patient care and staff effectiveness. - Knowledgeable of the basic medical procedures, terminology, etiology and disease process associated with the illnesses of the client population. CERTIFICATIONS/LICENSURES REQUIRED: + Licensure in the State of Michigan as a Social Worker (LMSW). PHYSICAL DEMANDS/WORKING CONDITIONS: + May be exposed to potentially infectious materials, bloodborne disease pathogens and hazardous waste. + May work in the client's home which is an uncontrolled environment. May need to lift, carry, bend, stoop or be otherwise physically active in the normal course of job duties. + May drive to and from clients' homes located in the Detroit Metropolitan area in varied weather and traffic conditions. May require a valid driver's license and a satisfactory driving record. May require rotating on-call responsibilities Overview Under the leadership of President and CEO Wright L.
