Job Description The primary purpose of this professional position is to assist individuals with developmental disabilities and their significant family members/circle of supports through the process of developing and carrying out an Individual Plan of Service based on the person---s expressed needs, wishes, and cultural preferences. Major job functions include connecting individuals with resources and services to encourage optimal independence, community experiences, and quality of life. Much of the work is performed independently and requires considerable knowledge of support coordination / case management techniques and developmental disabilities, nursing standards and best practice as well as knowledge of local, state, and federal regulatory standards affecting persons with developmental disabilities. The employee receives general supervision and works within established protocols and procedures in the form of regulations, rules, policies, and oral instructions. In addition, this position may be called upon to address the health care needs of individuals by screening, evaluating, coordinating and monitoring health care needs under established local, state and federal laws, standards and regulations. Registered Nurse Requirements Proof of COVID-19 vaccination is required for this position. Exemption process available. Possession of a Bachelor of Nursing degree. One year of experience, preferred Current Licensure by the State of Michigan as a Registered Nurse Current Michigan Driver---s License Certified in Emergency First Aid and Cardio-Pulmonary Resuscitation (CPR). Registered Nurse Responsibilities Major Areas of Responsibility: DEVELOPMENT AND MONITORING OF INDIVIDUAL TREATMENT AND/OR SUPPORT PLAN PROVIDE OPPORTUNITIES FOR TRAINING AND COUNSELING ASSIST INDIVIDUALS/FAMILIES IN ACCESSING RESOURCES PROFESSIONAL ASSESSMENT, TREATMENT AND MONITORING HOSPITAL AND SKILLED NURSING FACILITY DISCHARGE PLANNING CUSTOMER SATISFACTION Development and monitoring of individual treatment plan Identifies health care needs through assessment and coordinating with individuals, family, caregivers and physicians as applicable Completes thorough care plans that identify specific health needs with identified goals Evaluates individual---s progress towards their identified health goals Provide opportunities for training Assists individual, family and caregivers in accessing medical services as needed Provides education and guidance related to medical issues to encourage individual empowerment with healthcare needs Facilitates training and guidance to caregivers specific to meeting the needs and wishes identified in the individual---s treatment/support plan. Identifies and monitors safeguards to minimize risk to the individual in their home and community Assist individuals/families in accessing resources Assists individuals/families in exploring and accessing health services in the community Assists individuals/families in selecting and accessing health care providers Professional Assessment, Treatment and Monitoring Delivers nursing health assessments to assist in identifying and resolving individual---s health and safety concerns Monitors effectiveness and side effects of medications and treatment plans Provides health care recommendations Develops standards of care/treatment plans according to a person centered approach Develops community supports addressing health care needs for individuals with complex care needs Provides periodic review concerning health care treatment at frequency recommended in Individual Plan of Service (IPOS Monitors and evaluates medical charts, nursing care guidelines, health care plans, treatment and response as outlined in the IPOS Develops the amount, scope, frequency, and duration of nursing services based upon individual need and medical necessity Attends special consultations regarding individuals and their health care concerns Participates in individual planning and team meetings as needed/requested Provides training to caregivers, family members and individuals served related to specific health and/or safety concerns Provides nurse consultation regarding the health care needs of individuals served in the following manner: Interpret physician---s orders, link with other health care professions such as physician, care provider, schools, workshops, support team members, guardians, parents, hospitals and inpatient care programs as needed.
