Care Manager- LPNEPIC Primary Care's mission is to provide exceptional healthcare for our community. We are a Medical Management Company and a Multi-Specialty Practice treating patients from the beginning of life to the very end stages of life. Our network consists of Physicians, Psychologists, Exercise Physiologists, Physical Therapists, Care Coordinators, Medical Billers and much more. We look at all aspects that play a significant role in our patients everyday busy lives.

  • Must be local to Metro Detroit area for training and regular meetings, possible hybrid position will be considered.Skills:
    • Understand the EPIC Way, including FOVIS, how patients are seen, what services are available, the expectations for customer service and treatment of patients and employees.
    • Understand all EPIC policies, procedures, and protocols and be able to ensure that direct reports understand and follow them
    • Excellent communication skills, ability to build patient and caregiver rapport
    • Answer emails, messages in Teams, and messages in EHR within a timely manner
    • Ability to read and comprehend simple instructions, short correspondences, and memos
    • Ability to write simple correspondence that are easily comprehended by recipients
    • Ability to effectively present information in one-on-one and small group situations to internal and external customers
    • Always maintain confidentiality, including compliance with HIPAA
    • Ability to adapt to new policies and procedures as directed
    • Identify trends in patient health and provide basic clinical coaching to help improve status
    • Identify opportunities and areas of patient need to eliminate barriers to wellness
    • Natural sense of curiosity and ability to think beyond "checking the box"Responsibilities:
      • Educates patients about why it is important to come to their appointments, especially for preventative care.
      • Helps patients understand the Epic way
      • Ensures care coordination and follow-up with other provider offices when referrals to specialists are made
      • Requests referrals and authorizations when necessary
      • Assists with training other staff members as needed
      • Listens to inquiries from both patients and EPIC staff and takes appropriate action to serve both internal and external customers
      • Handles appropriate level of customer service, including documenting complaints in the patient's own words and escalating as necessary
      • Scheduling appointments as appropriate per the patient needs and the EPIC Way
      • Providing self-management education to ensure the patients have the tools they need to manage their chronic conditions at home
      • Assessing social determinants of health and barriers to wellness, making appropriate interventions to make optimal health and wellness accessible to the patient
      • Performing specific clinical interventions like care planning and medication reconciliations as needed
      • Responding to immediate needs for patient intervention
      • Managing other inquiries as assigned
      • Identifies gaps in care and proactively attempts to address/close gaps at every patient encounter
      • Refer patients as appropriate to remote patient monitoring for disease managementQualifications:
        • Licensed Practical Nurse (LPN)
          • 2-5 years of Care Management Experience
            • 2 years of direct Patient Care in a PCP, Internal Medicine or Specialty Care Office.
              • Telephonic or Telemedicine experience preferred
                • Compassionate and people oriented
